Here's a list of required documents for individuals applying for a national visa (D) with the right to work, along with their original documents and copies:
1. Completed visa application through the e-konsulat system.
2. Passport photo with a white background.
3. Valid passport (expiry date must be at least 90 days after the visa's expiration).
4. Medical insurance covering expenses of at least 30,000 Euros, valid from entry into Poland until the signing of the employment contract and enrollment in ZUS insurance (for a minimum of one month after entering Poland). 5. Work permit issued by the competent authority of the Republic of Poland (Voivodeship office).
6. Documentation indicating the applicant's residence in Poland, such as a hotel reservation, lease agreement for accommodation post-arrival, or another residency proof (property deed) with the precise address.
7. Flight ticket booking to Poland.
8. Preliminary employment contract signed by both parties, detailing remuneration, working hours, and job responsibilities (in Polish and/or English).
9. Documentation verifying the enterprise's operations where the applicant will work, including the National Court Register, current and previous year's income, income tax payments, and employee count.
10. Proof of adequate funds for traveling to Poland and covering expenses for at least the first month of stay, such as a bank statement for the last three months (attested by the bank) or traveler's checks.
11. Employer invitation letter, providing contact details of the issuer for consul inquiries. The letter confirms the future employee's enrollment in ZUS and health insurance, specifies the role, position, and salary of the foreign worker.
