To obtain a tourist visa for Poland with a Ghanaian passport, you'll need a passport valid for at least six months beyond your intended stay, as well as proof of onward travel from Poland. Visa application requirements:
1. Complete and legibly filled application form, signed by the applicant.
2. Two passports meeting the specified criteria.
3. Previous passports and a passport protocol.
4. Two up-to-date biometric photos meeting the specified criteria.
5. a) Health insurance worth €30,000 covering the entire stay in Poland; OR
b) Proof of medical insurance from the Social Insurance Institution (ZUS) in Poland.
6. Work permit from Poland.
7. Signed and stamped letter from the Polish company regarding future employment, including the name, surname, and position of the signatory.
8. Certificates and documents confirming vocational education and qualifications.
9. Social Security Document proving social security history and Social Security Registration Certificate from the last workplace (with barcode or signed and stamped).
10. Civil Status Register excerpt.
11. Criminal Record Certificate.
12. Military Service Certificate.
13. For applicants: Valid residence permit until the date of departure.
14. Copies of the biometric page of the passport and Polish or Schengen visas issued in the last three years.
15. Visa fee (non-refundable even in case of rejection).
